Paper work for documentary

This is the story board i created for my documentary. I created this storyboard to give myself an insight in what my documentary is going to include. My final documentary docent really match my storyboard because of different circumstances, For example transport and various different things, but i used every advantage i could have used to create my documentary.

This is an actor release form, i had to fill out one of these for many reasons, the main reason was to show the interviewee that this footage they are included in may have to go on various different networks and different media , it is also used to make the interviewee aware that no fee is payable for their performance.

I had to fill out a recce for each different places i filmed at, for example i shot kayleigh's interview at Hyde Clarendon sixth form college, on this sheet i included all the shooting information, lighting information, sound information and power information. You fill this out beforehand in order to have no mistakes when filming, it is much easier to have one of these. When you get to your location you can then just set up to the way you have described and film therefore it won't be time consuming.

I then had to fill in a risk assessment for each bit of filming i did, for example this is also Kayleigh's interview. A risk assessment is very important you choose your location and decide beforehand what would be a risk to you, the interviewee and the equipment, this way there will no damage to any property, and nobody will get hurt.

This is also all the paperwork i had to do for Kimberley's interview , this includes a risk assessment, an actor release form, a recce and also a risk assessment, all these documents are very important because without them you can't continue with your filming.

This is my shot list, this includes each individual shot that is included in my documentary and how long its duration is and also what type of shot it is, for example cut away or interview.

These were the dates i shot all my documentary, i did them on different days because it was very time consuming. I did this in order to make it easier for myself knowing where i was going and when i was going, therefore i didn't have to do any messing about.

This is my personal production log, i did this to explain what i filmed and how i filmed it, i described all the obstacles what got in the way and how i overcome them, i also explained that i had finished my documentary and what i would have done to make it better.
